Community Nurses are Registered Nurses working under the direction of a District Nurse Team Leader.

In line with the vision of 'My Life a Full Life', the Isle of Wight is a first wave Vanguard site with a focus on developing Integrated working across the whole Health and Social Care economy.

Community nursing is changing rapidly -we now provide all aspects of nursing care at home for many patients who would have traditionally remained in hospital.

The three locality teams work closely with our colleagues from EMH, Crisis Response, OT, Physiotherapy, GPs, Practice Nurses, Specialist Nurses, Mainland Hospital and outside agencies.

Worried that you do not have the necessary skills?

There is no doubt that you already possess a wealth of skills and knowledge that you can share with us.

We would expect you to have good interpersonal skills and be able to demonstrate that you are an evidence-based practitioner.

Clinical skills in all types of urinary catheterisation, syringe driver management, wound management and diabetes management are desirable, but not essential.

Support and training will be available to ensure a safe transition to working in the community setting.

A preceptorship programme is available for newly qualified nurses.

After a service reconfiguration there are a range of hours available (both full time and part time) in several posts.

The Integrated Community Nursing Service operates Island-wide, seven days a week, between the hours of 8.00am and 8.00pm. You will be required to work flexible shifts to meet the needs of the service. There also may be opportunities in the future to work on a rotational basis across the Business Unit incorporating the Emergency Department and the Medical Assessment Unit, and this will be discussed at interview.
